To submit an incident follow this link and complete the incident form so we can file a Worker's Compensation report right away.
Tip: For the quickest and most accurate support, this needs to be completed within 24 hours, or sooner, of the incident. If you are in need of immediate medical assistance, please go to the nearest medical facility.
Worker's Compensation is insurance we have to support our workers when they are injured on the job. Depending on your state, Worker's Compensation may cover your medical bills and may pay a portion of your lost wages from missed work. Once the form is completed and has been submitted, someone from our Workers Compensation team will reach out to you for a further detailed explanation of the incident.
